Replace x() by isset(), !empty() or defaults()

- Remove extraneous parentheses around empty() calls
- Remove duplicate calls to intval(), count() or strlen() after empty()
- Replace ternary operators outputting binary value with empty() return value
- Rewrite defaults() without x()
